Proudly Serving Alabama, Northwest Florida & the Gulf Coast
High-quality care comes from more than a single visit — it comes from understanding the people we serve. We focus on building strong relationships with the departments we work with across Alabama, Northwest Florida, and the Gulf Coast, so our care is informed, consistent, and meaningful. Our team understands your culture, crews know what to expect, and that familiarity translates into better outcomes at every exam.
Comprehensive Care for
High-Risk Professionals
Our proactive, comprehensive health surveillance program is purpose-built for the unique occupational hazards firefighters and law enforcement officers face every day.
Cancer Screenings
Targeted screenings for the occupational cancers most prevalent among firefighters and first responders, using a full suite of multi-organ ultrasounds, advanced imaging options, and comprehensive lab panels. Early detection is the single most effective tool we have.
Comprehensive Physicals
In-depth medical assessments focusing on cardiovascular health, pulmonary function, cancer indicators, and overall fitness for duty — aligned with NFPA 1580 (formerly NFPA 1582) standards and tailored to the occupational demands of firefighters and first responders.
Routine Follow-Ups
We don't stop at the exam. Our team conducts periodic check-ins between annual physicals to ensure abnormal findings are addressed, specialist referrals are coordinated, and every member stays on track. Our Nurse Navigator works directly with each first responder to provide continuous, personalized support.
Behavioral Health Evaluations
Evidence-based assessments for stress, sleep disruption, anxiety, depression, and trauma commonly faced by first responders. Proactive mental health support to keep first responders resilient on and off duty.
Why Screening Matters in Public Safety
Firefighters and law enforcement officers face elevated risks for cancer and cardiovascular disease. The research supports one conclusion — proactive screening saves lives.
Occupational Cancer LODDs
79.4% of IAFF members honored at the 2025 Fallen Fire Fighter Memorial died of occupational cancer (247 of 311).
IARC Classification
IARC classifies firefighting at the highest carcinogenic level — with sufficient evidence that it causes mesothelioma and bladder cancer.
Mesothelioma Incidence
A pooled U.S. firefighter cohort study found mesothelioma incidence 2.29 times that of the general population.
Duty-Related Fatalities
Sudden cardiac death accounts for about 45% of firefighter duty-related fatalities.
Fire Suppression Risk
During fire suppression, risk of death from coronary heart disease is estimated at 10 to 100 times higher than during non-emergency duties.
Coronary Calcium Detected
In one study of asymptomatic firefighters age 40+, 39% had measurable coronary artery calcium — hidden cardiovascular disease without symptoms.
Smith et al. (2018), Kales et al. (2007), Ashen et al. (2021)
Higher Cancer Mortality
A long-term Buffalo, NY police cohort found all-cancer mortality was 14% higher than expected among white male officers.
Hodgkin’s Lymphoma
In a U.S. police incidence study, Buffalo officers had 3.34 times the expected incidence of Hodgkin’s lymphoma.
Higher Colon Cancer
The updated Buffalo police mortality study found colon cancer mortality was 41% higher than expected.
Shorter Life Expectancy
A Buffalo, NY police cohort study found officers had a mean life expectancy 21.9 years shorter than U.S. males.
Avg. Heart Attack Death Age
In a 22-year analysis of duty-related law-enforcement illness deaths, the average age of death due to heart attack was 46.5 years.
Restraint / Altercation Risk
Compared with routine or non-emergency activities, officers’ risk of sudden cardiac death was 34 to 69 times higher during restraints and altercations.
Violanti et al. (2013), Violanti et al. (2023), Varvarigou et al. (2014)
The Return on Investment
Cost is often the first question departments ask. Investing in annual health screenings isn't just the right thing to do — it's a smart financial decision that more than pays for itself through reduced claims, earlier treatment, and fewer lost duty days.
Request More InformationReduced Workers' Comp Claims
Catching conditions sooner leads to less costly treatment and fewer extended disability claims.
Lower Healthcare Costs
Late-stage cancer treatment can cost up to 7× more than early-stage care — and that’s before accounting for extended disability, replacement staffing, and long-term benefits.
Fewer Lost Duty Days
Healthier firefighters mean better coverage, reduced overtime, and lower recruitment costs.
Recruitment & Retention
Departments that meet or exceed NFPA 1550 wellness standards signal that leadership invests in its people — a proven factor in attracting and keeping top candidates.
Built Around the Standard. Designed to Surpass It.
Every VITALExam is rooted in the NFPA 1580 framework for occupational medical programs. But we go far beyond what the standard requires — delivering advanced diagnostics, cancer-focused imaging, and behavioral health assessments that set a new benchmark for first responder care.
Emergency Responder Occupational Health and Wellness (Formerly NFPA 1582)
Our exams align with NFPA 1580, which now incorporates the former NFPA 1582 requirements — covering cardiac, pulmonary, vision, hearing, musculoskeletal assessments, and Chapter 13 medical conditions tracking.
Wellness-Fitness Initiative
Programs aligned with the IAFF/IAFC WFI for holistic firefighter wellness including behavioral health and fitness assessments.
Exam Components
Every VITALExam analyzes over 300 biomarkers and data points — spanning cardiovascular, pulmonary, oncologic, metabolic, and behavioral health domains — to build a complete picture of each first responder's health.

Commitment
Early detection. Consistent follow-up. Ongoing communication. We hold ourselves to a higher standard because the people we serve hold themselves to one too.

Trusted by Fire Service Leaders Throughout
Alabama & Northwest Florida
As a two time cancer survivor, I realize the importance of the services provided by VITALExam. Marci and her team provide an outstanding service that makes the health of our Firefighters a priority. The manner in which Marci and her team provide this service is both caring and professional. They help me ensure the health and wellness of our department, which in turn helps us provide better service to our community.
VITALExam's first responder body scan was truly life-saving — they detected cancer in my body early, giving me the chance to take action before it was too late. Their thorough and professional screening process provided me with answers I didn't even know I needed, and I am forever grateful. This program means everything to me because it not only gave me a fighting chance but also reaffirmed the importance of prioritizing my health as a first responder.
VITALExam has become a vital part of Tuscaloosa Fire Rescue, helping us keep a closer eye on the health of our firefighters. By catching potential health issues early, they not only help prevent serious medical emergencies but also support the overall wellness of our crew. This proactive care ensures our firefighters are always ready for the job while protecting their long-term health and well-being.
The total health and well being of public safety employees are vitally important to Marci and her team. You're not just a client, they treat you as if you're family and your outcome is important to them. They not only run the initial tests, they also follow up and give you a personal information portal that you can access anytime. VITALExam genuinely invests in you and your health journey.
This was our first year using VITALExam, and we couldn't be more impressed. Their team provided exceptional care and professionalism throughout the entire process. From comprehensive physicals to behavioral health support, they went above and beyond to ensure our firefighters were fully taken care of.
VITALExam's focus on the total well-being of first responders truly stands out. They understand the demands of our job and provide the tools and attention needed to keep our department healthy, resilient, and ready to serve our community.
VITALExam has the most knowledgeable and professional staff I've ever worked with. This is the third year our department has used VITALExam, and each year they have identified potentially serious health issues that I was able to address early, before they became major problems. That alone is invaluable.
Beyond the exams, they provide clear, practical guidance on improving overall health. They take the time to explain as little or as much about your personal health as you're comfortable with, never rushing and never talking over you. Quite honestly, they have done more to improve my overall health than my primary care physician.
Highly recommend VITALExam to any department or individual serious about their health.
